Union City: Your First Visit

New patients are encouraged to bring the following to their first visit to the Union City Dental Care Center.

If you have records and X-rays from another dental office, please bring them with you to your first appointment. If you have been referred by another office for specific procedures, please bring a referral note with the dentist's name, number and address.

For patients with special medical conditions, it is highly advisable to bring your current medical laboratory results with you as well as a list of all the medications you are currently taking.

If you routinely take "antibiotic prophylaxis" one hour prior to your regular dental appointments, whether here or at other dental offices, please take them as prescribed before coming. If you're not sure if you need antibiotic prophylaxis, you can still come to your first appointment please do be aware that we may not be able to complete the entire comprehensive exam for you on that day. If you've never had a need for antibiotic prophylaxis prior to dental appointments, then this should not be a concern for you.

Please give yourself plenty of time to travel here on your first visit, especially if you are unfamiliar with the area. Parking in our lots and on the street is available.
