Overcoming Difficulties in Local Anesthesia

There are a number of reasons why difficulties in administering local anesthesia can occur in a dental practice. The goal of this course is to provide the dental practitioner with the tips and confidence to master the techniques needed to properly administer local anesthesia through a combination of lecture, demonstration laboratory and hands-on clinical participation. Special emphasis will be placed on mandibular block anesthesia and anesthetizing accessory innervation to mandibular posterior teeth.

You Will Learn

  • Correct usage of various local anesthetics
  • Paresthesias and dysesthesias related to the use of articaine and prilocaine local anesthetics
  • Anesthetizing substance abusers and others who have a history of being difficult to anesthetize
  • Determining the cause of incomplete anesthesia
  • A systematic approach to obtain complete anesthesia

Clinic Demonstration and Participation

  • Participants will be divided into small groups to simulate or directly practice these techniques:
  • Review range of osseous and soft tissue landmarks
  • Learn techniques of successful mandibular anesthesia
  • Gow Gates mandibular block anesthesia
  • Correct landmarks for inferior alveolar anesthesia njections for blocking accessory nerves that often innervate mandibular teeth
  • Techniques for anesthetizing maxillary quadrant of teeth with fewer injections
  • Painless nasopalatine injection (really!)
  • Nearly atraumatic greater palatine injection

Faculty

James S. Dower, Jr., DDS, MA, is a former associate professor in the Department of Restorative Dentistry and director of the local anesthesia curriculum at Pacific, where he taught for 35 years. He has presented local anesthesia courses at regional, state and national dental meetings and his research articles have appeared in national and international journals and publications. He also holds a master's degree in educational and counseling psychology.

Bernadette Alvear Fa, DDS, is an assistant professor in the Department of Restorative Dentistry at Pacific and is director of the local anesthesia curriculum. A graduate of Pacific, she was also a clinical instructor at University of Illinois, Chicago, for two years before joining the Pacific faculty in 2010.
