CARE (HIV) Program

Pacific's CARE clinic coordinates oral  health care services to patients with HIV. It is one of the nation's largest Ryan White CARE-funded dental programs, serving both adults and children with HIVand AIDS.

Hours of Operation

Patients are seen by appointment or on an emergency basis..
Monday - Friday 9:00 a.m. - 1:00 p.m., 2:00 p.m. - 3:00 p.m.

All clinics are open January - December and closed during holidays and exam weeks.

How to become a patient

Call the CARE Program for registration packet at (415) 929-6648. In order for new patients to qualify for Ryan White funding, you must bring the following documents to your first appointment.

  1. Requirement: You must be a resident of San Francisco County.
    Documentation: Provide proof of residency in San Francisco, for example, rental agreement, utility bill, official government letter, etc. (No P.O. Boxes)
  2. Requirement: You must have personal identification.
    Documentation: Provide picture identification, such as driver's license or California ID card.
  3. Requirement: You must be HIV seropositive.
    Documentation: Provide letter of HIV diagnosis.
  4. Requirement: You must have a personal income below $30, 000 per year.
    Document: Provide proof of income, for example, tax return, public assistance, award letter, etc.
  5. Requirement: You must supply a blood laboratory report (due within 6 months).
    Documentation: Provide a copy of a recent complete blood count with differential, platelets, and lymphocytes.
  6. Requirements: You must be under the care of a physician, or in the process of obtaining a physician.
    Documentation: Provide a physician consultation form which is signed by your doctor. These forms are provided by the CARE office.

For people who have Medi-Cal insurance, please see the following:

  1. Requirement: You must use your private dental insurance for all allowable benefits.

Please Note: Any elective procedure billed to private insurance which is not a CARE approved service will require that the patient pay the co-payment or portion of the bill not paid by insurance.

Basic dental care available at no cost for persons meeting the above criteria.
